To begin, please complete your health risk assessment by clicking on the link to the left. You will also need to register for a lab draw or submit labs to be analyzed with your questionnaire to give you a complete health profile.

Before you begin, you may want to review the FAQ’s regarding your privacy and personal information that we keep confidential, and if you need help deciding which lab tests are right for you, click on the Lab Tests link to find more information on the different tests offered.

Wellness Credits

NOTE: If you attend a health fair, we will have your lab results and will take a blood pressure measure there, so you won’t need to submit anything. If you cannot make it to a health fair site, or will be attending a community fair, then you’ll have to submit those results and a blood pressure independently; don’t forget that if you are submitting labs independently, you should check to make sure all the necessary values are there, and that you also have a recent blood pressure measure. Instructions, as well as a Fax Cover sheet, for submitting lab results will be available through your FIBS registration portal on iNet.

Your lab results will be analyzed in connection with your questionnaire, and a complete wellness profile report will be sent to you within 2 weeks after a draw site, or within a month after labwork submission
